Acute trauma
Overuse

Age >40
Post-menopause
More likely on dominant side
Smoking
Posture
Fam hx
Repetitive lifting or OH activities
Trauma:  fall on shoulder or dislocation (more common in younger).

Fatty infiltrates

A

atrophy, fibrosis, and fat accumulation around muscle.
More common in elderly.
Moderate infiltrates 3yr after tear.
Severe infiltrates 5yr after tear.
ideally want surgical tx before moderate infiltrates occur.
