PRE-OXYGENATION *** (5)
Open pop off valve (1)
Apply tight mask seal (1)
Assure O2 flow meter > 5 liters/min (1)
3 forced vital capacities within 30 seconds with 100% O2 (1)
3 minutes of 100% O2 with a goal of >85% end tidal oxygen conc. (1)
Ask if pre-treating with non-depolarizer if using succinylcholine (1)
Complete machine check (1)
Complete equipment set-up (laryngoscope, ETT, airway, suction on at head of table) (1)
Complete medication check (1)
Apply pulse oximeter (1)
Apply ECG leads (1)
Apply NIBP (1)
Obtain baseline vital signs *** (4)
Apply cricoid pressure *** (4)
Administration of Drugs *** Maintain cricoid pressure (2) Administer propofol (2) Administer muscle relaxant immediately after induction agent (2) NO positive pressure ventilation (1) Consider taping eyelids (1)

Intubate (1)
Correct ETT depth (1)
Cuff inflated (1)
Connect to anesthesia machine circuit (1)

Auscultate bilaterally (mid-axillary line & epigastrum) (1)
Release cricoid only if intubation is confirmed (4) Tape ETT (1)
