What do you look at first on a CXR?
Mediastinal Contour
- tracheal central?
- aortic arch
- left pulmonary artery
- left atrium
- left ventricle
Where should the hear sit?
2/3 left (no more than 1/2 chest cavity)
1/3 right
What do you look at with regards to lung shape?
Costophrenic angels - V shape
Cardiophrenic angles
What are the technical details of a CXR?
M or F Old or young Good inspiration? Good penetration? Rotation?
PA or AP?
What would show good inspiration
Diaphragms at least at level of 6th ribs anteriorly
How can you judge rotation?
Medial ends of the clavicle
Spinous processes
Give a chest summary for a normal patient
Trachea is central
Mediastinum is not displaced
Mediastinal contours and hila appear normal
Lungs appear clear with no pneumothorax
The bones are soft and tissue seems normal
What are common review areas on a CXR?
Apices
Periphery of lungs
Under hemidiaphragm
Behind the heart
What can cause mediastinal shift?
Pleural effusion pushes away
Tension pneumothorax pushes away
Collapse pulls trachea towards
Causes of cardiomegaly?
Carditis LVH Congestive heart failure Athleticism Pericardial effusion
What are sign of LVH?
Kerley B lines
Bats wings
Alveolar shadowing
Upper lobe diversion
What are Kerley b lines?
Tiny horizontal from pleural edge
Look like fine hair
What are bats wings?
Type peri-hilar haziness
