Can occur from:
Tube shift (MC)
Using focused grid
Could also be due to a bent grid holder
FFD out of range = bilateral grid cut off
Parallel grid = bilateral grid cut off

Note:

NOTICE that one side is lighter than the other…notice the different color of air
