What is diagnosis of RA

A

Family history, pain history and examination of joints

X-ray - joint damage
DAS28 to monitor initial severity
CPR and ESR - inflammation makers (will be raised)

A

1st line methotrexate

Methotrexate SERIOUS SE

A

STOP and report ASAP SAME day
Sore throat, bruising, mouth ulcers - blood disorders

Nausea, Vomiting, dark urine - liver toxicity

Shortness of breath and dry cough, fever - respiratory effects

STOP bone marrow suppression - reduced white blood or platelets

STOP taking if have infection requiring antibiotics and report any signs of infection to GP

A

NSAIDS (increased risk of toxicity)
Alcohol ( increased risk of liver toxicity)
Trimethoprim, Amoxcillin
Live vaccines
AVOID ppl with chicken pox / measles
Pregnancy

Orange urine and stain tears and lenses yellow (normal)

STOP and REPORT ASAP
Sore throat, brushing, bleeding, fever, purple rash - blood disorder

Leflunomide CI and cautions

A

Pregnancy - contraception for 2 years and 3 months for men after treatment ( wash out protocol can reduce this)

STOP taking if infection needing antibiotics
