What are the muscles of the floor of the mouth?
Mylohoid, Geniohyoid, Stylohyoid, & Digastric Belly (Anterior/Posterior)
What are some structures involved in swallowing? What do they do?
Lips: hold food in the mouth
Tongue: propels bolus back
Hyoid: move up and forward, closing upper airway
Larynx: closes from bottom up, prevents food from going into airway
Epiglottis: folds over to protect the airway
Teeth: mastication of the food
UES: allows bolus into esophagus
Velum: closes to protect nasal, enables pressure in pharynx
What is medication that has a big effect on swallowing?
Antibiotics - can cause confusion, disorientation, dysarthria, dysphagia, and weakness
What are components to look for in a case study?
Chief complain, present/past illness, medications, trauma, type of dysphagia (food, liquid, pills), onset/progression, family/social history
What are some benefits of a chin tuck?
For a patient with oral or pharyngeal dysphagia.
- Helps with decreased base of tongue control
- Widens vallecula and narrows airway entrance
- Protects trachea opening from aspiration by moving airway below the tongue base and epiglottis
What is the head tilt used for, reason behind using it, and how you would execute it?
Used for a unilateral oral impairment to aid in control of the bolus, normally tilt to strong side, gravity pulls bolus to side of oral cavity with greater muscle tone
-Instruct the patient to tilt their head to strong side (model this)
What are 3 swallowing maneuvers that you could recommend?
Supraglottic Swallow
Effortful Swallow
Masako Swallow
Explain the Supraglottic Swallow.
- To close chords before/after the swallow
- Take a deep breath and hold it, hold while swallow, then cough
- “Breath in, hold it, swallow, then cough”
Explain the Effortful Swallow.
- To increase posterior motion and pressure of tongue base, to reduce residue
- Squeeze hard with all your muscles when you swallow
- “Pretend you have a golf ball stuck in your throat! Work hard to get it down!”
Explain the Masako Swallow.
- To increase superior constrictor (glossopharyngus), increase retraction of tongue base and anterior bulge of posterior pharyngeal wall
- NOT done with food, hold tongue between teeth while you swallow
- “Bit down on your tongue, then swallow”
