Quiz 1 Flashcards
lymph nodes infected in strep infection
cervical
lymph nodes infected in mono infection
cervical
nodes can get enlarged with bug bits to scalp
post auricular and occipital
facial findings with hyperthyroidism
Bulging eyes (exophthalmos), face is thin from weight loss, swelling neck (goiter). Pts have a bright eyes stare. an a lid lag. Hyperthyroidism can occur in pregnant women from increased secretion of human chorionic gonadotropin (HCG) with a molar pregnancy. Women with hyperthyroidism are at increased risk of complication from preeclampsia and thyroid storm.
facial findings with hyperlipidemia
xanthelasma, arcus senilis
dacrostenosis
blocked tear duct
ciliary flush
Conjunctival vessels are most dilated at the corneal edge. Could be a sign of corneal injury/infection, acute iritis, or glaucoma.
symptoms of retinal detachment
floaters, flashes of light, a curtain effect across vision that doesn’t go away, sudden loss of vision (medical emergency)
testing for corneal abrasion
a fluorescein stain is applied to the eye, adhering to abrasions only, and illuminated with a Wood’s lamp
systemic presentation of Hypothyroid disease
Hypo….bradycardia, decreased systolic and increased diastolic pressure, decreased intensity of heart sounds, dry, coarse, cool skin, non-pitting edema, loss of hair, impaired memory, mixed hearing loss, somnolence, peripheral neuropathy, carpal tunnel, peri-orbital puffiness. Decreased DTR
URI/allergies effect on tympanic membrane
cause retraction of TM
Wharton’s duct
Base off the tongue, duct of the submandibular gland. Open on either side of the lingual frenulum.
5 parts of treatment plan
Non pharmacological intervention Pharmacological intervention Education needs of patient and family Follow up Referral
5 parts of well-child history
head circumference weight and length, nutrition and elimination, development stage, immunization, birth history
conjunctivitis types and types of drainage
allergic - mucoid
bacterial - purulent
viral - clear
three subjective and objective findings of otitis externa
S: otalgia, otorrhea, loss of hearing
O: swelling, erythema, otorrhea, edema of canal
tympanometry
test for fluid behind eardrum or eustachian tube blockage - it is an objective test of middle-ear function. Helpful in making a diagnosis of otitis media.
strawberry tongue
glossitis which manifests with hyperplastic (enlarged) fungiform papillae, giving the appearance of a strawberry. White strawberry tongue is seen in early scarlet fever, turns red after 4-5 days
correct order of history
Allergies, medications, immunizations, illness/surgeries, LMP
bell’s palsy cranial nerve findings
CN VII - facial paralysis or droop, hyperacusis (increased sensitivity to hearing) present
N-B (Nylen Barany test)
Also known as the Dix-Hallpike Test or head over bed test - diagnostic maneuver used to identify benign paroxysmal positional vertigo (BPPV). from a sitting position the patient then rotates 45 degrees and is laid down quickly with the head hanging at 20 degrees. observe eyes for 45 seconds for nystagmus.
nystagmus as an abnormal finding
vestibular dysfunction, rhythmic osscilation of the eyes. Disorders of the labryrinth and the cerbellar system? Lesion from the brain stem, cerrebellum, vestibular system, or along the busual pathways in the the cerebral hemispheres.
how to reinforce DTR’s
isometric contraction of other muscles which may increase reflex activity (use when symmetrically diminished or absent)
most common type of headache
tension headache
Brudzinski
One of the physically demonstrable symptoms of meningitis is Brudzinski’s sign. Severe neck stiffness causes a patient’s hips and knees to flex when the neck is flexed.
red flag for sciatica
Numbness, decreased/abnormal sensations to the the thigh and lower extremity. Radiating pan in one or both legs from lower back to thigh and leg. Difficulty walking.
finding for L3/L4 herniation
reduced knee jerk DTR
findings for carpal tunnel syndrome
Tinel - Phalen. Possible findings… Aching, tingling and numbness in second, third and fourth volar fingers is a positive Phalen’s sign.
Tinel…Percuss lightly over medial nerve and you will get tingling or electrical sensation, this positive sign of Tinel suggesting carpal tunnel.
5 techniques to assess low back pain
Straight leg raise, percussion, range of motion, deep tendon reflexes, palpation of spine muscles, lateral bending, abduction and adduction. Crossed straight leg raising sign.
5 parts of neuro
sensory, DTRs, strength, CNs, mental status, coordination
5 general observations in general survey of PE
Apparent state of health… whether acutely ill, frail or robust, vigorous
Level of consciousness
Signs of Distress….Cardiac of resp., distress, pain, anxiety or depression
Skin color and obvious lesions.
Dress, grooming and personal hygiene
Facial expression….watch for eye contact, is natural. Star….hyperthyroidsim; flat or sad affect
Odors of the body of breath
Posture, gait and motor activity.
Speech, pattern and hearing.
three subjective and objective findings of otitis media
S: otalgia, otorrhea, tinnitus
O: fever, bulging red TM, change in light reflex
hyperthyroid
Tachycardia or atrial fib, increased systolic and decreased diastolic pressure, warm, smooth, moist, skin. Tremor and proximal muscle weakness. Graves disease, eye signs such as lid lag, stare and exophtalmus.
