Purpura, etc Flashcards
what is purpura
non-blanch, pink/purple macules/patches or papules caused by extravasated RBC in skin or muc memb. “blood in skin”
purpura causes
- coag/clotting prob
- leaky /abnormal vessels
- other (trauma eg)
macular/non palp purpura
non inflamm
palpable purpura
vasculitis
petechiae
small <3 mm red or purple dots. Usually on dependent areas of body. no pain usually
who will have petechiae: hemaphilia or chemo?
pt w chemo because thrombocytopenic
think petechiae, think ___
PLATELETS (low or dysfunctional)
are there non-platelt related petechiae?
yes! things that increase cap fragility or allow leaking
example of petechiae
scurvy
ecchymosis
non palp purpura!
larger >5 mm
any part of body
could be painful or not, tender
eccymosis is due to…
abnormal coag! not platelet defects
hyper OR hypo coag
causes of purpura
thrombocytopenia _ inf/inflamm/truama
abnormal paltelet fn + inf/inflamm/truama
infection
anticoag + truama (DIC, renal/hep dysfn, anticoag med, vitK defy)
poor dermal support + trauma (actinic damage, amyloid, steroid atrophy, Ehlers-Danlos, trauma, scurvy)
linear purpura
thrombocytopenia + trauma
henoch-schonlein purpura
assoc w kidney dis bc vaculitis
watch for pregnant women too
PALPAPLE
classification of vasculitides
size of vessel!
size of vessel determines what in vasculitides
clinical findings
classification
small vessel vasculitis
Henoch Schonlein purpura infections drug rxns A/I malig-assoc vasc idiopathic/ H/S rxn
polyarteritis nodosa
big vessel vasculitis
medium vessel vasculatis
not as much palpable purpura.
most scary form of purpura?
retiform purpura!!
what is retiform purpura
distinctive ecchymosis w “netlike” pattern
due to vasc ischemia bc underlying thrombotic disorder
widespread = purpura fulminans
infection, congenital coag def (protein C defy)
acute thrombotic event
if fever, infection esp meningococcemia! eek.
purpura fulminans
widespread retiform purpura
