Pulmonary HTN Flashcards
How do you define pHTN
mPAP > 20 mmHg in > 3 month old at sea level
How do you define pre-capillary pHTN?
mPAP > 20 and PCWP < 15
How do you define post-capillary pHTN
mPAP > 20 and PCWP > 15
What two gene mutations are associated with heritable pHTN?
TBX4 and BMPR2
What combination of physiologic states is associated with early and aggressive PVD?
High flow state and high pressure state leading to sheer stress and wall tension
Describe the diagnostic algorithm for pHTN per the AHA/ATS guidelines
Echo > PFTs and sleep study > V/Q scan > cath
Describe the treatment algorithm for pHTN per the AHA/ATS guidelines
Needs cath prior:
If AVT + -> CCB
If AVT -
– Low risk : Endothelin receptor antagonist or PGE5i
– High risk: Triple therapy with prostacyclin analog
