How is the D-dimer test interpreted?
Negative test excludes PE, but positive test does not confirm it
What is first-line imaging for PE?
CTPA
Management of a large pulmonary embolism (5)
Oxygen 100%
Morphine 10mg IV + anti-emetic
Consider thrombolysis e.g. with alteplase
Heparin + warfarin for 5 days
Warfarin for at least 3 months (INR aim 2.5)
Prevention of PE (3)
TED stockings
Early post-op mobilization
LMWH prophylaxis (fragmin)
When is thrombolysis indicated for massive PE?
Circulatory failure (i.e. hypotension)
