Classification of pneumothorax?
Primary if there is no underlying lung disease; secondary if there is
Management of primary pneumothorax?
If rim of air <2cm and patient not SOB- discharge
Otherwise- aspirate. If fails- chest drain
Management of secondary pneumothorax?
All patients admitted for at least 24 hours; if >2cm, chest drain; if <2cm aspirate
Definition of pneumothorax?
Collection of air in the pleural cavity
How is a tension pneumothorax formed?
Development of a one-way valve allowing movement of air into the pleural space
Management of tension pnuemothorax?
High flow oxygen
Aspirate using a 16-18G cannula inserted into the second anterior intercostal space in the mid clavicular line
Presentation of a patient with pneumothorax?
Mnemonic- PTHORAX
Pleuritic pain Tracheal deviation Hyper-resonant percussion Onset sudden Reduced breath sounds (and SOB) Asymptomatic if small X-ray shows collapse
