what are the symptoms of PE?
- acute dyspnoea
- pleuritic chest pain
- haemoptosis
- syncope
what are the signs of PE?
- cyanosis
- raised JVP
- pleural rub
- pleural effusion
- tachypnoaic
- tachycardic
what should you look for if you suspect a PE?
- look for signs of DVT
when should you perform a d-dimer?
when a high probability they dont have a PE- exclude cause
does a positive test prove a PE?
no
what investigations are done to diagnose PE?
- wells score: looks at probability
- CT pulmonary angiography -1st line
- leg ultrasound
what is atypical causes of PE?
right ventricular thrombus (post MI)
septic embolus from IE
neoplastic cells
what are the treatments for PE?
- LMWH
- alteplase if massive embolus
- warfarin
which drug is given for prevention of a PE?
heparin- dalteparin
when should LMWH be stopped?
when INR >2
where does the embolus wedge?
into one of pulmonary artery ( not bronchi!)
what signs are observed in a massive PE?
- gallop rhytmn
- right ventricular heave
- split 2nd heart sound
- raised JVP
what is the classical presentation?
- 10 days post op whilst straining to stool
what would the ECG should in a massive PE?
- sinus tachycardia
what would the ECG should?
type 1 resp failure
