A

Progressive dysponea and signs of right sided heart failure

This is once you have excluded all other secondary causes.

A

Warfarin (prophylactic anticoagulant)
Oxygen if hypoxic
Pulmonary vasodilators such as PDE5 inhibitors (sildenafil) and IV epoprostenol
