What is required to diagnose chronic bronchitis?
chronic bronchitis lasts 3 months in each of two successive years
paroxysmal phase of untreated pertusissis may last 2-3 months, so make sure to differentiate
What is acute bronchitis?
a lower respiratory tract infection involving the large airways without evidence of pneumonia, in the absence of COPD
What is the clinical presentation for bronchitis?
cough 1-3wks
consitutional symptoms
rhonchi with or without wheezing
rhonchi clears with coughing
What are the clinical manifestations for pnuemonia
cough, dyspnea
fever
tachypnea
rales
What are the CURB/CRB criteria?
new onset confusion
severe tachypnea
hypotension
age >65
What is the clinical presentation of COPD?
productive cough featuring sputum production for >3months for 2 years
chronic dyspnea
increased AP diameter of chest
prolonged expiration on auscultation
hyper=resonance with percussion
What is the clinical presentation of pertussis?
immunization can wear off in 5-10yrs
coughs following mild uri sx with conjunctivitis
worse at night
post-tusive emesis
feel well between bouts of coughing
What is the clinical presentation of Asthma?
episodic wheezing, dyspnea, cough
worse night and early morning
allergies and swollen mucosa may accompany
prlonged expiration and diffuse wheezing
What should be examined during an acute exacerbation of asthma?
office treatment albuterol pre- and post-peak flow measurement
pulse ox
listen to lungs as well as peak flow measurement
What should be taught and reviewed at each visit with the asthma pt?
daily peak flow
correct MDI use
trigger avoidance and ID
What happens when an asthma pt falls into the yellow zone?
What happens when pt falls into red zone?
step up meds
retest in 20-30min
goal to regain gren zone
contact physician or get help
What will the ABG show in ARF?
PaO2 < 60mmHg or PaCO2>45
What is CPAP?
continuous positive airway pressure
provides constant form of positive airway pressure ventilation
What is BiPap?
bi level positive airway pressure
provides lower level of airway pressure with exhalation
What type of rehab is helpful in pts with chronic respiratory failure?
pulmonary rehap to include nutritional management, rehab therapies, pt education, medication, O2 delivery
