PFT in asthma

A

FEV1 and FVC decreased, but FEV1 more than FVC so FEV1/FVC ratio is also decreased. FEV1 inc with > 12 % with beta2 agonist (albuterol) dec in FEV1>20 % with metacholine. Inc in diffusion capacity of lung for carbon monoxide (DLCO)

Sarcodosis definition

A

Chronic granulomatous disease characterized by noncanseating granulomas. Often involved multiple organ systems, lungs are almost always involved. 75 % occurs before 40 yrs, Inc risk in african americans.

A

bilateral hilar adenopathy, skin anergy, elevated ACE, hypercalciuria,hypercalcemia, definitive diagnosis: transbronchial biposy.

Tx

sarcoidosis

A

most resolve spontaneously in 2 yrs and do not need treatment. If treatment; systemic corticosteroids

A

2 months with isoniazide (INH), rifampin, pyrazinamide, etambutolol/streptomycin, then 4 months with IHN and rifampin.
