what is sarcoidosis
idiopathic chronic multi systemic inflammatory non-caveating granulomatous disease
RF for sarcoidosis
females
black americans
Northern Europeans
20-40 y
sx sarcoidosis
50% asx
pulmonary - dry cough, dyspnea, rales, may have chest pain
lymphadenopathy (swelling of lymph nodes)
skin - erythema nodosum (classic), lupus pernio (most specific)
eyes - anterior uveitis, dry eyes, vision loss
cardiac - cardiomyopathy, arrhythmias, heart blocks
rheumatologic - arthralgias, fever, malaise, wt loss
neurologic - cranial nerve palsies esp facial nerve, diabetes insipidus
what is Lofgren syndrome
associated w sarcoidosis
triad of erythema nodosum + bilateral hilarity lymphadenopathy + polyarthralgia w fever
dx sarcoidosis
CXR
PFTs
Tissue biosy

what will CXR show for sarcoidosis
best initial test
bilateral hilar lymphadenopathy classic
what will PFTs show for sarcoidosis
restrictive pattern classic
normal or increased FEV1/FVC
decreased lung volumes - RV, FRC, TLC
decreased DLCO
what will tissue bx show for sarcoidosis
noncaseating granulomas
what will labs show for sarcoidosis
increased ACE levels
hypercalciuria
hypercalcemia
increased vitamin D
tx for sarcoidosis
asx - observation
sx - oral corticosteroids
different stages of sarcoidosis (you determine staging from CXR)
stage 1 - b/l hilar adenopathy
2 - b/l hilar adenopathy + interstitial lung disease
3 - interstitial lung disease only
4 - fibrosis (restrictive dz)
