DBT used to reduce suicidal behavior in Borderline personality d/o
(i) Individual component- discuss issues recorded on diary cards
- self-injurious/suicidal behaviors first, follows by therapy-interfering behaviors
(ii) Group component- once weekly
- mindfulness
- interpersonal effectiveness
- emotional regulation
- distress tolerance skils
Indications for behavioral therapy
Phobias, sexual dysfunction, compulsions, enuresis
2 Methods of behavioral therapy
(i) Conditioning
Differentiate classical and operant conditioning
Classical = association of stimuli (salivate at bell)
Operant = learning by reward (mouse learns to press lever to get treat)
What drug is an example of aversion therapy
Antabuse disulfram
aversion therapy = type of behavioral therapy useful in addiction and paraphilias
Indications for biofeedback
HR, pulse etc shown to pt to help control
- migraines
- HTN
- chronic pain
- bronchial asthma
- incontinence
Indications for supportive psychotherapy
Useful during transient decompensation (ex: adjustment d/o)
Differentiate systemic desensitization and flooding
Systemic desensitization: anxiety hierarchy- start w/ minimally invasive exercises and progress
Flooding (real stimuli) and implosion (imagined stimuli)- stimulus removed only when relaxed, relaxation techniques
Contraindications for psychoanalysis
Psychotic pts
-or ppl w/ severe cluster A and B personality d/o
-not indicated for ppl who have problems w/ reality testing

(a) Id (unconscious) is present at birth
(b) Ego (mediator btwn id and external environment) develops after birth
(c) Superego (moral conscience) development begins at age 6
Differentiate unconscious, preconscious, and conscious
Unconscious = out of one’s awareness, involved primary processing (primitive, pleasure seeking w/ no regard for logic or time, seen in children and psychotics)
Preconscious = memories that are easy to bring into awareness, but not unless consciously retrieved
Conscious = current thoughts and secondary process thinking (logical, organized, mature, can delay gratification)
Describe the mechanism, what category is it?
(a) sublimination
(b) reaction formation

Defense mechanisms = unconscious processes, used by the ego to relieve anxiety
(a) sublimination (mature): satisfying socially objectionable impulses in an acceptable manner (ex: unconscious urge to physically control ppl => become prison guard)
(b) reaction formation (neurotic): doing the opposite of an unacceptable impulse (ex: man in love w/ his married coworker insults her)
(c) repression (neurotic): unconsciously preventing a thought from entering consciousness
(d) suppression (mature): purposely ignoring an unacceptable impulse or emotion to accomplish a task
(e) projection (immature): attributing objectionable thoughts or emotions to others (ex: husband attracted to other women believes his wife is having an affair)
What defense mechanism is often seen in borderline personality d/o?
Splitting- labeling ppl as all good or all bad
“you’re cool but I hate the evil attending”
Indications for psychoanalysis
Psychoanalysis- resolve unconscious conflicts by bringing repressed experiences into awareness. Insight oriented
- cluster C personality d/o
- anxiety d/o
- OCD
- problems coping w/ life events
Describe the concept of CBT
Triangle of of thoughts, feelings, and behaviors
- work to change behaviors to change the other 2 (behavioral)
- work to change thoughts to change the other 2 (cognitive)
