Bite marks on dorsum of hand due to gagging in bulimia
Bulimia has hypokalemia

Do not treat with NSAIDs
Serotonin syndrome treatment and drug to avoid
AVOID acetaminophen
DC serotonergic agent: SSRI, SNRI, regale, Zovirax, at johns wort, Wellbutrin, lithium, cocaine, tryptans
Tx with benzo lorazepam
Bipolar I
At least 1 manic episode with or without major depression. Depression is usually present more than mania (manic episode lasts at least 1 week, grandiose, no sleep, flight of ideas, etc)
Bipolar II
At least one episode of depression
at least one hypomanic episode (lack of social/cognitive impairment)
NO HISTORY of mania
Cyclothymia
numerous periods of hypomania and depression for at least 2 years that switch off
Bipolar treatment

Valproate (Depakote)
Carbamezapine (Tegretol)…Leukocytopenia
Atypicals (Risperidone, Seroquel, Zyprexa, Geodon)
Atypical Antipsycotic that causes agranuloctyosis (need constant WBC count)

Schizophrenia
need to have sx for at least 1 month of delusions, hallucinations (positive sx) , disorganized speech, catatonic behavior, negative sx (flat affect, alogia, poor grooming, anhedonia)
Tx with 1-2 mg Lorazepam (Ativan)
Schizoaffective Disorder
Mood disorder with psychotic features. Psychosis can occur without mood disturbances. Delusions or hallucinations occur in the absence of depression for more than 2 weeks
Brief Psychotic disorder
psychotic symptoms with sudden onset and full remission within one month ( more than 1 day less than 1 month)
Delusional disorder
one or more delusions over a month. Lack of impairment in functioning. Lack of manic episodes or depression.
Schizophreniform disorder
Schizophrenia at least one month but no more than 6 months. Impaired social and occupational functioning but not required like with schizophrenia.

impaired memory and anterograde amnesia
Cocaine intoxication
tachycardia, pupil dilation (mydriasis), diaphoresis, nausea
Opioid Toxicity

Opioid Toxicity Antecdote
Naloxone (Narcan) to increase respirations
Dysthimic disorder
chronic depression where sx are milder and last more than 2 years (major depression is loss of interest in activities, vegetative state etc for more than 2 weeks)
Antidote for Benzodiazapine OD
Flumazenil (Romazicon)
Vomiting should be induced with gastric lavage and activated charcoal in pt who is not comatose
Axis Dx.
I: acute sx that need tx (mood disorders, schizophrenia, depression, panic attack, etc)
II: Personality disorders and intellectual disability (retardation, learning disability, OCPD)
III: Medical disorder relevant to problems (kidney failure, diabetes)
IV: Psychosocial disorder (Divorce, homeless)
V: Global assessment functioning (0-100)
