Psychological Therapies: Family Interventions Flashcards
What is the use of family interventions?
used with family members as well as patients, other therapies only involve patient
What is the rationale behind using family interventions?
schizophrenia is maintained by communication/emotional problems and high levels of EE in family, EE not sole cause of schizophrenia but affect course of illness
What are the aims of family intervention?
Provide family with practical coping skills, to reduce EE levels within family to thus reduce relapse rates
What are the processes involved in family intervention?
educate family on schizophrenia, improve communication and cooperation, adjust expectations of family members, reduce EE levels, recognise early signs of relapse, expand family social networks: talk to other parents
Who conducted a Cochrane review of family intervention for schizophrenia?
Pharoah 2012: vs standard care, 53 RCTs,
Who conducted a Cochrane review of family intervention compared with standard care for schizophrenia?
Pharoah 2012: 53 RCTs, may decrease frequency of relapse and hospital admission, encourage compliance with drugs, improves general social impairment and levels of EE
Who conducted a meta analysis into family intervention compared with standard care and schizophrenia?
NCCMH 2009: 32 studies, reduction in hospital admission and symptom severity during and up to 24m after, intervention: 26% control: 50% relapse rate
Who has issues with randomisation? What study issues the problem of randomisation?
Pharoah: treatment effect may be overestimated due to poor methodological quality. Wu 2009: Chinese RCTs on disease reviewed, only 10% did randomisation properly
Who found that families may not want family intervention as a treatment of schizophrenia?
McCreadie 1991: 63 relatives of 52 schizophrenics offered similar to intervention, half agreed, quarter actually attended
What is the issue with all family intervention studies?
Patients always receiving standard care = drugs, doesn’t show direct effect (is intervention effective without drugs?)
Who is family intervention not possible for?
schizophrenics who live alone/not close with their families
