Criteria for conduct disorder
Repetitive & persistent pattern of behaviour which violates the basic rights of others OR major age-appropriate social norms/rules
Across 4 domains:
- aggression toward people or animals
- destruction of property
- deceitfulness or theft
- serious violations of rules
at least 3 features present for at least 1 year
Tx for Conduct disorder
Treat any psych disorder e.g., ADHD
Target major modifiable RF at early age
Parent programmes
- individual (books) or group
- focus on behavioural therapy techniques e.g. clear rules, modelling good behaviour
Cognitive problem solving skills training
Multi-systemic therapy
Mentoring
Criteria for oppositional defiant disorder
Childhood (<11yo) psych diagnosis of behavioural problems not due to substance abuse or psychotic disorder
pattern:
- angry / irritable mood
- argumentative / defiant behaviour
- vindictiveness
lasting at least 6 months
Criteria for learning disability
significantly reduced ability to understand new or complex information OR learn new skills
reduced ability to cope independently
impairment started before adulthood - with lasting effect on development
Levels of learning disability
Profound <20
Severe 20-34
Moderate 35-49
Mild 50-70
Difference between LD and learning difficulty?
Difficulty e.g. dyscalculia, dyspraxia, dyslexia
With adequate support, will not impact IQ
Tx of Tic disorder
Stress management + re-assurance
Medical
- clonidine (adrenergic alpha 2 agonist)
- antipsychotics
Tx of depression
Mild depression persisting >4 weeks = CBT
Antidepressants only in combo with therapy
- prescribed by specialist
- require MDT discussion
- Fluoxetine = only med licensed
Childhood self-harm management
Any <16yo with self harm has to be reviewed by CAMHS before discharge
Can be admitted to Paeds ward to allow this to happen if necessary
