What are some negative characteristics of a tooth that has been prepped for an extra coronal restoration?
Sensitivity due to dentine exposure
Loss of coronal seal of endodontically treated teeth
Reduction of occlusion - destabilisation
What are the characteristics of a good temporary restoration?
Stabilise occlusion
Reduce sensitivity
Good marginal seal
Good contour / no plaque retentive factors
Cleanable by patient
Maintain aesthetics
Prevent microleakage
Non-irritant
Low temp rise during setting
Adequate setting and working time
Good strength and wear resistance
What material may typically be used for a temp restoration?
Protemp - chemically cured bis-acrylic resin
What materials may be used to take an impression of a tooth for a provisional restoration?
Addition cured silicone putty in a stock tray
- can be reused as resistant to tearing sand dimensional change
- expensive
Alginate is cheaper but can’t be reused
Vacuum formed stent
Modelling wax
Method for putting in temp?
Sectional impression
Prepare tooth
Take impression of prep for final restoration
Syringe protemp / material into impression and seat
Remove before full polymerisation
Check prep and fit of temp and thickness
Cement temp
How may wax ups be used to do a composite build up?
Get mounted casts of teeth through impressions
Ask lab to do wax up, they will do a wax up and duplicate cast
Get a vacuum formed stent produced over the wax up
Use stent to produce a temporary restoration
When may a preformed crown be used for a provisional?
How is this done?
Trauma cases where no impression taken prior to tooth prep or damage
Select shell slightly larger than prep and trim it back until its dimensionally accurate for needs, does not bed into gingiva
Fill shell with protemp and seat over tooth to allow polymerisation
Remove and check fit of provisional, and make adjustments if necessary
Cement the provisional with temp luting cement
How may a missing tooth be restored PROVISIONALLY?
Use an Essix retainer with a Pontic in situ. The Pontic will fill the gap and retainer hold in place
Can use an acrylic RPD such as a spoon denture
What advice should be given to someone with a temporary in place?
Maintain good oral hygiene with brushing 2-3 times daily
Clean interdental 1-2 times per day but be wary of floss as it can unseat the temp restoration!
Risk of gingival inflammation due to increase in gingival crevicular fluid
