Provisional Extra-Coronal Restorations Flashcards
What do provisional extra coronal restorations have a role in?
- immediate & long term health of tooth
- health of supporting structures (e.g PDL)
- success of definitive restoration
What are the characteristics of a good provisional restoration?
- reduced sensitivity of tooth
- good marginal fit
- be well contoured
- cleansable & maintainable by patient
What are the consequences of placing a poorly fitting/contoured provisional restoration?
- patient unable to clean
- caries
- gingival inflammation
When you place a provisional restoration, what must you check before letting the patient leave?
- no OVD changes
- no major masticatory/speech issues
- will not cause drifting/tilting of tooth
What are the desirable characteristics of a provisional material?
- non irritant
- low setting temperature
- dimensionally stable
- adequate working time
- adequate setting time
- adequate strength & wear resistance
- good aesthetics
What material is used in custom resin provisional crowns?
Chemically cured bis-acrylic composite resin
- protemp plus
What instrument is used to ensure there has been sufficient reduction of a tooth prior to placement of a custom resin provisional crown?
Iwanson Gauge
What is the methodical steps for placement of a custom resin provisional crown?
- sectional impression
- prepare tooth for chosen restoration
- mix bis-acrylic comp resin
- syringe material into sectional impression of prepared tooth
- relocate impression in the mouth & ensure fully seated
- remove before complete set
- remove and flash & ledges on restoration
- check marginal fit & occlusion & aesthetics
- cement in mouth
What are the different types of preformed provisional crowns?
- aluminium
- stainless steel
- polycarbonate
What are the problems with placement of preformed provisional crowns?
- unlikely to fit accurately
- large bank of crowns needed which is costly
What are the methodical steps for placement of a preformed provisional crown?
- select shell slightly larger than preparation
- trim to ensure it seats fully & not bedding into gingivae
- fill shell with protemp
- seat over tooth
- allow polymerisation
- remove & check fit/trim tidy if needed
- cement using a temp luting cement
Using which instruments can an old crown be removed?
- WAMkey
- Safe Relax/Anthrogyr
- Sliding hammer
What can be used as provisional replacement of missing teeth?
- bridgework temporisation
- acrylic RPD
- essix retainer with pontic
