- LUTS - obstruction so voiding problems
- Haematuria, haematospermia
- Pain - back, perineal, testicular
- ED, weight loss, anorexia
What is found on DRE?
asymmetrical hard nodular enlargement w loss of median sulcus
where does it commonly metastasis to?
bones - causing pain
What investigations can be done?
- PSA
- DRE
- Transurectal USS + biopsy
- MRI/CT + bone scan for staging
- Gleason scoring
What is Gleason scoring?What scores signify low, intermediate, high risk
grades tumour by biopsy
Low: ≤6
Intermediate: 7
High: 8-10
What is the management of low risk localised prostate cancer?
Active surveillance or:
- radical prostatectomy
- radiotherapy: external beam and brachytherapy
What are management options for localised advanced prostate cancer?
Hormone therapy ]
Radical prostatectomy
Radiotherapy: EB + B
What are important risks of radiotherapy rx for prostate cancer?
increased risk of bladder, colon and rectal cancer
what is a common complication of radical prostatectomy
erectile dysfunction
What is rx of metastatic prostate canceR?
- synthetic GnRH agonist e.g. Goserelin, cover initially w anti-androgen to prevent rise in testosterone
- Anti androgen - cyproterone acetate
