A

Vision (could be 3 or 5 years) –> Goals (Defined as metrics) –> Initiatives (f.ex. add support for 2 new languages) –> Releases (grouping of functionality and features that are grouped and released at same day) –> Epics –> Stories and requirements.

Intro (summary of what the features are for, the why, which metrics you want to improve, early wireframes maybe, MVP vs ideal state, legal requirements), product requirement (the feature must X), design requirements (the designer should be involved here, wireframes could be here), engineering requirements (technology domain).
Product manager usually do 1 and 2.
