What equipment is needed for a surgical airway
A
- cleaning equipment
- 10 blade scalpel
- size 6 orotracheal tube - lubricated
- boogie
- end tidal CO2 monitoring
- tie

Describe step-by-step process of surgical cricothyroidotomy
A
- clean site
- identify landmarks
- stab incision longitudinally until you feel a ‘pop’
- rotate 90 ‘ with sharp edge towards patient feet
- swapping hands provide traction towards yourself keeping blade perpendicular
- insert boogie with it facing towards the blade
- realign so it is in line with trachea and move it 10-15cm in the trachea
- remove scalpel, stabilise boogie, and railroad orotracheal tube (soze6) over boogie
- remove boogie and attache end tidal co2 and ventilation circuit
- monitor co2 trace x5 and auscultate
- secure in place with tape/ tie

What modification may be needed for patients with a high BMI when performing a surgical cricothyroidotomy?
A
A vertical incision should be made over the overlying skin/fatty tissue to expose the landmarks of the airway
(from sternal notch to chin)
