The consultant has recommended that warfarin is restarted. They have restarted at the patients usual dose. Explain if it is appropriate for the patient to still have enoxaparin prescribed.

Concomitant heparin therapy affects the results of control tests and should be discontinued at least six hours before the first test is carried out.

Recommend an appropriate dose of enoxaparin for this patient. (Should this dose be prophylactic i.e. 40mg or treatment i.e. 1.5mg/kg? Will the dose need to be adjusted for renal function?
