  • FP10-REC are used by OOH providers to record items supplied directly to a patient and not dispensed through a community pharmacy, submitted directly to NHSBSA for reimbursement

  • Pharmacist can charge any amount
  • RX for a POM must be retained for 2 years from the date of last sale/supply

  • Drug name, strength, quantity, direction are not LEGALLY required
  • Written indelibly
  • Age and or DOB if under 12
  • Patient name and address
  • Date (must be dispensed within 6 months)
  • Signed in ink by prescriber
  • Type of Prescriber
  • Prescribers address (name not legally required)

Repeat Prescriptions

A
  • Can be repeated as indicated by prescriber (eg. repeat x3 = prescription is dispensed 4 times in total
  • If number of repeats isn’t stated, prescription can only be repeated once (dispensed twice), unless it is for an oral contraceptive, that can be on repeat x5 so in total 6.

Record of Supply

A
  • Must be made in the POM register (must be a book or electric)
  • Entry must be made on the day or following day
  • POM register retained for 2 years from date of last entry
  • Exemptions of POM register:
  • Prescriptions for OC
  • Schedule 2 CD drugs (Separate CD register record would have been made., however good practice to still record)
