Prioritisation Flashcards
What do you say when given mutliple patients
PATIENT SAFETY and CLINICAL NEED are my absolute priorities, according to which I will need to prioritise.
– briefly summarise four patients –
I understand I am in a position where I have three potentially unwell patients who all need to be assessed urgertly. As I cannot attend to all of them at once, I will need to prioritise on clinical need and safety and also delegate.
To determine this, I will request further information from the nurse via SBAR handover + NEWS trend
What do you say when picking your patient
Based on the information i have, Patient X is my top priority
This is because they are …
I am particularly worried about [TOP THREE DIFFERENTIALS]
This patient has significant potential to deteriorate without immediate assessment
What else do you do when ON YOUR WAY TO ATTEND CHOSEN PT
Call the nurse - request:
- Fresh observations
- prepare notes and drug chart
- (as necessary) bloods (bottles), ECG, prep ABG, oxygen if hypxoxic
- if patient deteriorates > put out 2222 call
): Contact other team member (SHO/Reg) and inform of situation - 2 unwell patients -
- – if pt 2 also severely unwell: could they go see pt 2 in meantime and I will join once pt 1 is stabilitised
- – if pt 2 not so seveerely unwell: will review one now but may need your help
What do you say (brief summary before A>E) once you arrive at pt bedside
I would perform very quick assessment from end of bed as I arrive to see if pt severely visibly unwell
Immedialtely CLINICALLY ASSESS and STABILISE patient using A>E approach, then TREAT in line with LOCAL GUIDELINES
