What criteria are included in evaluating or weighing each completed task for process?
Is it life-threatening or potentially life-threatening if the task is not done? Would another patient be endangered if I do this now or leave this task for later? Is this essential to pt or staff safety? Essential to the medical or nursing plan of care?
Levels of priority setting?
Airway, breathing, cardiac/circulation, vital signs (ABC’s plus V)
Mental status changes, untreated medical issues, acute pain, acute elimination problems, abnormal lab results, risks
Other health problems: long term issues in health education, rest, coping, etc.
Maslow’s hierarchy of needs?
Survival needs Safety and security Affiliation (love, relationships) Self-esteem Self-actualization
What things should be thought of when it comes to time management?
What is the relative importance of each task? How much time will it require? When must it be completed? How much time and energy has to be devoted to it?
How to avoid crisis management?
Gather materials, cluster activités, do not ignore intuition, clarify before beginning task, stick to the schedule, limit interruptions, finding the fastest way
Reassigning of responsibility for the performance of a job from one person to another
Delegation. Accountability remains with the delegator. The person to the task was delegated also has responsibility and accountability to accept the task and for their own action in carrying out the act.
RN responsibilities that cannot be delegated?
Initial admission client assessment. Decisions and judgement on client outcomes. Formulation of client plan of care. Interventions requiring professional nursing judgement, decisions, and skills. Evaluation of client care decisions and judgements
Delegation also requires what?
Supervision, which is the provision of guidance and direction, oversight, evaluation, and follow-up by the licensed nurse for accomplishment of a nursing task delegated to nursing assistive personnel.
Five rights of delegation?
Right task Right circumstance Right person Right direction/communication Right supervision/evaluation
The right task if the one that in the RN’s best judgement can be what?
Safely delegated for this pt, given his current condition and future preferred outcomes, if there is a competent individual to perform it
Explain right circumstances?
Know the rules and regulations of your practice area. Know the pt’s condition. Know the preferred pt results. Sometimes a best judgement must be made
What must the RN know in order to fulfill the idea of the right person?
Must know the licensure, role and preparation of each member of the team. Job description of coworkers and scope of practice. Identify strengths and weaknesses of team members.
What must happen with temp staff when it comes to choosing the right person?
Ask about previous experiences. Ask if they understand their work duties. Assign a resource person to assist them.
Four C’s of right direction/communication?
Clear, concise, correct, complete
Explain right supervision?
The RN is accountable for the total care of the pt throughout the shift. RN may not actually perform the task but just oversee the ongoing progress and results obtained.
