OPQRST
Onset - Did it start suddenly or over the course of a few hours/days?
Provocation - What provokes the pain, or causes it to get worse?
Quality - What kind of pain is occurring? (Ache, sharp, stabbing, throbbing, etc.)
Region & Radiate - Where is the pain and is the it moving distally or proximally?
Severity - On a scale of 0-10, how intense is the pain?
Time - When is the pain at its worst?

Burns
Lacerations
Swelling
Tenderness
Instability
Crepitus
Distal Limbs: PMS
Pulse - Presence/quality (cap refill)
Motor - Can you move your fingers/toes?
Sensory - What digit am I touching?

Signs & Symptoms - What’s bothering you?
Allergies - Do you have an allergies and did you come into contact with it?
Medications - What meds do you take, and have there been any changes?
Past Medical History - What medical conditions do you have?
Last Oral Intake - What and when was the last thing you consumed, including meds or supplements?
Events Prior - What happened to cause this problem?
Types of Patient Positioning
Lateral: Lying on side
Supine: Lying on back
Recovery: Halfway between prone and lateral
Fowler: Lying on back with body elevated at a 45-60 degree angle
Semi-Fowler: Lying on back with body elevated less than 45 degress
Trendelenburg: Lying on back with legs elevated higher than the head and body on incline

LOR: AVPU
Alert - Eyes open and able to verbalize coherent thought
Verbal - Eyes closed but patient responds to commands
Pain - Patient only responds to painful stimulus
Unresponsive - No response
Rapid Body Survey
Head/Neck:
- Skin Check
-Bruising around eyes/ears
-Fluids leaving any facial orifice
- C-spine palpation
Chest:
- Paradoxical breathing (Clavicle and lateral rib cage)
-Other Respiratory distresses
Abdomen/Pelvis:
-Expose abdomen and look for internal bleeding
-Fracture check by gently compressing and gapping pelvis
Back:
- When possible, put hand in EXPOSED lumbar to palpate spine
Lower Extremity:
- Fracture check by shearing femur in 2 places
- Wet check leg
- Distal PMS
Upper Extremity:
- Wet check
- Distal PMS
