Isoniazid (INH), Rifampin (Rifadin), Ethambutol (Myambutol), Pyrazinamide (PZA)
Which primary anti-TB meds can be given for prophylaxis?
INH, Rifampin
Impt Information for Primary Anti-TB meds:
- Use all 4 in combo
- Adjust after C&S results
- Airborne iso for at least 2 weeks
- Monitor for compliance
- PT ed: take as ordered at same time daily
- entire prescription should be finished even if feel better
- May need to take w food to decr GI upset
- Notify provider for s/s fatigue, yellowing of skin or eyes, or hot, painfully swollen joints
- photosensitivity may occur, wear sunscreen
Isoniazid (INH)
MOA: inhibits cell wall synthesis and interferes w metabolism
CAUSES VIT B6 DEF (peripheral neuropathy), LIVER TOXICITY
Contraindicated w/ use of CLOPIDOGREL
A/E: hepatitis, neuropathy
Can be used prophylactically
Rifampin (Rifadin)
MOA: inhibits protein & RNA synthesis
A/E: RED-ORANGE OR BROWN SECRETIONS, makes birth control INEFFECTIVE, hepatitis, abd pain, diarrhea, heartburn
Can be used prophylactically
Ethambutol (Myambutol)
MOA: inhibits protein synthesis
A/E: OPTIC NEURITIS and BLINDNESS, hepatitis
Pyrazinamide (PZA)
MOA: may inhibit lipid and nucleic acid synthesis
A/E: hepatotoxicity, GOUT
PPD Test
test for TB raised after 48-72 hours= positive redness is not indicative of positive X-ray to diagnose current infection If have the vaccine, PPD will be positive Blood draw can be used for those pts ^^
Antitubercular (not primary) Med
Bedaquiline (Sirturo)
Indication: MDR-TB (in conjunction w other meds)
Monitor LFTs
A/E: h/a, chest pain, nausea, QT prolongation, arthralgia, joint pain
TAKE W FOOD to incr absorption
