Describe cognitive errors

What are some common prescribing errors?

What should you always check when prescribing?
Patient age, weight, renal and hepatic function
Concurrent diseases, lab test results, concurrent medications, allergies
Med / surgical / family history
Pregnancy

What is the CHM?
Commission on Human Medicines
- advises ministers on matters relating to human medicinal products
- advises licensing authority
- considers representations by an applicant of MA holder
- promotes collection and investigation of information relating to ADRs to human medicines
Describe the SMC
Scottish Medicines Consortium
Established by Scottish government 2001
Principle remit to make decisions on cost-effectiveness of new/existing pharma products in respect of their use in NHS Scotland
