What three things constitute a pressure ulcer?
Pressure (compression of blood vessels)
Shear (stretched skin via gravitational pull)
Friction (generating heat, leads to blistering, skin tags and tears)

Skin is normal

Skin is intact, but reddened + doesn’t lighten when touched
Describe stage 2
Superficial, ulcers with loss of layers of epidermis skin
Describe stage 3
Full thickness skin loss, a deep crater

Skin loss of epidermis + dermis, great deal of damage to fascia, muscle, bone, tendon or joint capsule

Thin casing of connective tissue that surrounds and holds every organ, blood vessel
