3) reduced consciousness
4) sensory & autonomic impairment: incontinence
5) extremes of ages
6) malnutrition
clinical presentation of pressure ulcers
1) stage 1
- abrasion of epidermis
- irregular area of tissue swelling
- X open wound
2) Stage 2
- extend through dermis
- open wound
3) stage 3
- extend deep into subcutaneous fat
- open sore/ulcer
4) stage 4
- muscle & bone
- deep sore/ulcer

adjunctive measure for pressure ulcer
1) debridement of infected/necrotic tissue
2) local wound care
- normal saline, X harsh chemicals
3) relief of pressure
- turn/reposition every 12 hrs
