Pregnant Health History Flashcards
What are some things to consider when taking a health history from a pregnant patient?
- Ask about a confirmation of pregnancy
- Has the pt. had a urine pregnancy test, and when?
- When as the LMP?
- Has she had an ultrasound?
- Ask about any symptoms of pregnancy:
- Any missed periods, Breast tenderness, N/V, fatigue, or urinary frequency
- Concerns/Attitudes toward pregnancy
- Planned/desired?
- Who is father of baby?
- Family support?
- Past OB history
- Alcohol/smoking/illicit drug use
- Family history
- Postpartum contraception
- Plans for breast feeding
What past medical conditions should you explore in a pregnant patient?
Abdominal surgeries, hypertension, diabetes, cardiac disorders, asthma. hypercoagulability states, mental health disorders, HIV, STI’s, abnormal Pap smears can all affect pregnancy
What should you ask a pregnant patient in regards to their past OB history?
- # of previous pregnancies
- How many were term, preterm, spontaneous, terminated, and # of live births
- Any complications?
- Diabetes, hypertension, preeclampsia, preterm labor
- During birth?
How do you document pregnancy history?
- G = Gravidity (# of pregnancies)
- T = Term deliveries (# of babies birthed past 38 wks)
- P = Preterm deliveries (# of babies birthed before 37 wks)
- A = Abortions/miscarriages
- L = # of live births
What is gravidity?
The number of times that a woman has been pregnant
What is parity?
It is the number of times that a women has given birth to a fetus with a gestation age of 24wks or more, regardless of whether the child was born alive/stillborn
