What is the pathogenesis of pemphigoid gestationis?
- anti basemennt membrane zone serum facro:
- C3 deposition along the dermal-epidermal junction.
- post patients have antibodies TO BPAG2(180)
What are the clinical features of pemphigoid gestationis?
- classically presents during late pregnancy
- abrutp intesely pruritic urticarial lesions on the runk
- pemphigoid like erupsion sparing face,mucous membranes, palms and sole.
some patients may have spontaneous resolution during the later part of gestation only to flare.
- the symptoms may “skip pregnancies” in 5% of patients.
- new borns may be affected up to 10% of the time but the disease is mild- self limited.
What effects does pemphigus gestationis have on the newborn?
- increased risk of prematurity
- tendency for small-for-gesational-age in neonates.
What are the differential diagnosese of pemphigoid gestationis?
- PUPP
- allergic contact dermatitis
- drug eruptions
what is the treatment of pemphigoid gestationis?
- 5mg/kg with the risk of lower birth weight infancts and placental calicifications.
- alternatives including dapsone, doxyclicin/minocyline +/- nicotinomide, cyclosorine).
What patients is PUPP usually seen in?
- primiparous women
- rapid and excessive weight gain
- multiple-gestation
What are the clinical features of pregnancy?
- lesions start during the second or third trimester with discrete papules on extensor sufaces. Lesions may or many not be follicular.
What are the clinical features of autoimmune progesterone dermatitis?
- recurrent cyclical flares of dermatitis –> corresponds to the luteal phase of the menstrual cycle.
- lesions can look like urticaria to papulovesicles to erythema multiforme-like
