1-at clouds where temps are above 0, rain drops form because condensed water droplets collide with each other and increase in size, heavier droplets travel upwards at a slower rate than smaller ones and a chain reaction is established
2-ice particles must be present in upper part of cloud before precip can begin, normally only happens once temp falls below -10, ice particles will grow ravidly by deposition and collisions with super-cooled water droplets, once too heavy, they fall and grow in size

1-light
2-light-moderate rain
3-moderate-heavy rain
4-heavy rain
5-very heavy rain, hail possible
6-very heavy rain and hail, large hail possible
