Must be identified by the donor identification band containing the donor id, and/or the donor identification band correlating to the OPTN computer system.

Must be verified by both the onsite recovering surgeon and another qualified health care professional.

Must be verified by both the onsite recovering surgeon as well as another qualified healthcare professional
