Histocompatibility Flashcards
When should a lab report HLA typing on a deceased kidney, kidney-pancreas, pancreas, or pancreas islet donor?
Prior to organ offers
When should a lab report HLA typing on a deceased heart, heart-lung, or lung donor?
Prior to final acceptance, if required by the transplant program
When should a lab report HLA typing on a deceased liver donor?
Within the time period specified by the transplant program
If a critical discrepancy in HLA typing is found post-procurement what must the OPO do?
Notify and provide
supporting documentation to all accepting transplant programs within 24 hours
What must a laboratory do in a HLA antibody screening? (2 components)
1.Report anti-HLA antibodies identified to the candidate’s requesting provider
2. Use at least one solid phase immunoassay using purified HLA molecules
General crossmatching requirements (4 listed requirements)
1.Perform a crossmatch according to the terms specified in the written agreement between
the laboratory and the OPO or transplant program
2. Crossmatch potential donor T lymphocytes to identify class 1 anti-HLA antibodies
3. Crossmatch potential donor B lymphocytes to identify class 1 and class 2 anti-HLA antibodies
4. Use a crossmatch technique with increased sensitivity
How long should a lab preserve specimens from a deceased donor for subsequent histocompatibility testing after transplant?
5 years!
Which organ’s allocation may be denied based on a discovery of cytomegalovirus (CMV)?
Intestine
