What things should be considered during pregnancy in different parts of the history?
Medical History: Pre-existing medical condition Congenital Rubella Toxoplasmosis Listeria Infection

Folic Acid Supplements
Social History: Smoking Teratogenic medication Alcohol and Recreational Drugs Eating Liver Swordfish and tuna
How does smoking impact pregnancy?
Reduces birthweight and is also associated with an increased risk of
miscarriage, still birth and sudden infant death syndrome (SIDS)
How does Folic Acid Supplements impact pregnancy?
Reduces the risk of neural tube defects (low-dose
supplementation recommended for all women planning a pregnancy, higher
dose recommended for women at inc risk e.g. if they have a close relative with
a previously affected fetus/if they have diabetes/using anticonvulsants)
How does Pre-existing medical conditions impact Pregnancy?
Diabetes, epilepsy, hypertension – may
be monitored and management changed if needed
How does Teratogenic medication conditions impact Pregnancy?
e.g. retinoids, warfarin and sodium valproate
How do alcohol and recreational drugs impact pregnancy?
(opiates, cocaine)
How does congenital rubella impact pregnancy?
preventable by maternal immunisation before
pregnancy
How does Toxoplasmosis impact pregnancy?
exposure should be limited by avoiding undercooked meat
and cat litter (use gloves if handling)
How does Listeria Infection impact pregnancy?
can be acquired by eating unpasteurised dairy products,
soft ripened cheeses e.g. brie, camembert, pates and ready-to-eat
poultry (unless thoroughly re-heated)
How does eating liver impact pregnancy?
should be avoided because it has a high concentration of
vitamin A
How does swordfish and tuna impact pregnancy?
should avoid eating swordfish and limit tuna, due to
high levels of mercury. Limit oily fish intake as may contain pollutants
How does maternal obesity affect the mother?
inc risk of miscarriage, gestational diabetes and pregnancy-induced
hypertension
How does maternal obesity impact the foetus?
inc risk of stillbirth, congenital abnormalities, macrosomia and neonatal
mortality
How may obese mothers impact their children later on?
