Pre-operative & Intraoperative Flashcards
what are some important things to remember for a child having surgery?
- child & parents often feel anxious about surgery, teaching must first address this anxiety
- psychological preparation of both child & parents aimed at reducing a child fears abut the procedure & consists primarily of providing health teaching and oppurtunites for therapeutic play
- do not downplay a family’s fears, but allow the child & family oppurtunites to express their concerns as part f teaching/learning process
what kind of anesthetic do most children have?
most children who need a surgery receive a general anesthetic rather than a local or regional anesthetic as might be used with adults because this minimizes their fears of intrusive or mutilating procedures and because children who are not yet adolescent are not mature enough to cooperate adequately during surgery if they are not fully anesthetized.
what is the goal of the preoperative period
- for the patient to be as healthy as possible
- every attempt is made to address risk factors that otherwise may lead to postoperative complications and hinder recovery
Patient education specifics for the preoperative period
- procedure: what to expect and where to go
- Preparation requirements such as…
- NPO for 6-8 hrs beforehand
- Skin prep instructions and equipment (usually chlorehexidene- check for allergies)
- pt given directions for their usual medications
- bowel prep instructions and equiptment
- post-op exercises and what to expect
what are some of the preoperative plans for the surgical pt in nursing sense
- risk for infection will be minimized, pt aware to shower with chlorehxidiene scrub at HS & AM prior to surgery, pt will not shave surgical site
- client will be physically and emotionally prepared for surgery: IVs, medications, pre-op scrub, NPO, voided
- Risk factors will be identified and the surgical team will be made aware of
- pre-op checks, paperwork & diagnostics will be complete & correct, any concerns or omissions will be reported to the surgical team
- client will have knowledge related to surgery and what to expect before and afterwards. Deep breathing and coughing and ROM, splinting, mobility leg exercises will be learned and practiced
what kind of genetic considerations do nurses caring for surgical pts need to take into account
malignant hyperthermia
what is malignant hyperthermia?
disease that causes a fast rise in body temp and severe muscle contractions when someone with the disease gets general anesthesia, It is passed down through families
-in history find out if pts family had malignant hyperthermia or any bad or serious reactions in an OR that may have resulted in death
Why does nutritional status need to be assessed preoperativley?
-optimal nutrition essential in promoting healing and resisting infection
