Using Shoulder and Endurance to support performance in areas of occupation
Palpate
A-C joint & S-C joint
Scapula landmarks (spine, inferior angle, vertebral border) & Measurement of scapular alignment
Bicep tendon/ Supraspinatus, Infraspinatus/ Teres minor & Subscapularis
Provocative tests:
Empty Can
Bring the arms up to 90 degrees in the scapular plane (PROM)
Internally rotate (empty can motion)
Apply force down and instruct patient to resist force
Pain or weakness indicates injury in supraspinatus or long head of the biceps
Provocative tests:
Drop Arm
Abduct arm to 90 degrees (PROM) pronated
One hand on the shoulder, one under forearm
“Hold it there, then slowly lower it down”
Positive = unable to hold it/lower it slowly
Tear of supraspinatus
Provocative tests: Hawkins
Shoulder and elbow flexed to 90 degrees followed by forced internal rotation
Positive: pain
Testing supraspinatus/long head of biceps
Provocative Tests: Neer/Forced Forward Flexion
Stabilizing scapula from upwardly rotating, forced flexion with internal rotation
Positive: pain
Tests supraspinatus or long head of biceps
Provocative Tests: Biceps Speed Test
Shoulder is flexed to 90 degrees, elbow extended and forearm supinated
Resistance is applied to the distal forearm in the direction of extension
Positive: pain or weakness over long head of biceps
Tests biceps tendon
