A
Occurs <24hrs
Affects 5-7% deliveries

Most common cause of primary PPH?
A
Uterine atony (90%)
Others = genital trauma and clotting factors

Risk factors for Primary PPH?
A
Previous PPH Prolonged labour Pre-eclampsia Increasing maternal age Polyhydramnios Emergency CS Placenta praevia or accreta Macrosomia Ritrodine (used as a tocolytic)

Management of PPH?
A
ABC
IV syntocinon 10 units or IV ergometrine 500 mcg (constricts uterine arteries)
IM carboprost
Others:
B-lynch suture
Ligation of uterine and internal iliac arteries

What can be performed as a life saving measure?
A
Hysterectomy

Secondary PPH
A
> 24hrs - 12 weeks
Due to retained trophoblastic tissue or endometritis
