Pharmacologic Management of Uterine Atony (Uterotonic Agents)
A
-IV oxytocin: 40-80U in 1L saline
-IM methylergonovine: 0.2 mg
(contraindicated in patients with HTN)
-IM carboprost (PGF2α analog): 0.25 mg
(contraindicated in patients with asthma)
-PR misoprostol (PGE1 analog): 800-1000μg
-Tranexamic acid

Surgical Procedures for Uterine Atony
A
- Uterine packing or uterine balloon tamponade
- Compression sutures (e.g., B-Lynch suture)
- Surgical ligation of uterine or internal iliac arteries
- Uterine artery embolization (injection of thrombogenic material)
- Last resort: hysterectomy
