• Collection of blood in the subcutaneous layer
of the tissue of the perineum
• Injury to the blood vessels during instrumental
delivery or during repair of episiotomy
PERINEAL HEMATOMAS
What are the 4 types of perineal hematomas
> VULVAR HEMATOMA
VAGINAL HEMATOMA
VULVOVAGINAL HEMATOMA
RETROPERITONEAL HEMATOMA
When the hematoma is about
5 cm or less, no need for
treatment.
• Usually hematoma is
absorbed for about 3-4 days
or up to 6 weeks.
SMALL HEMATOMAS
• When the hematoma measures
about to 10 cm or more and is
increasing in size, the patient
may have to be returned to the
delivery room for incision or
bleeding vessel ligation.
• If episiotomy incision line is
opened for drainage, gauze
packing is placed.
• Be sure to record the packing.
LARGE HEMATOMAS
is a general term used to describe infection of the genital tract after delivery.
• Another leading cause of maternal mortality.
• Recovery depends on a multitude of factors:
• Virulence of the evading organism
• General health of the woman
• Portal of entry
• Degree of uterine involution
• Presence of lacerations
PUERPERAL FEVER OR INFECTION
The Causative agent are usually found in the normal flora
• Group B streptococci
• Escherichia coli
• Staphylococcal infections – toxic shock syndrome
• It may be introduced during labor and
delivery
INFECTING ORGANISMS
Refers to an infection of the
endometrium, the lining of the uterus.
•
Bacteria gains access to the uterus
through the vagina and enter the uterus
at the time of birth or postpartum.
• It is associated with chorioamnionitis
and CS birth
