Goals of post ressucitation care
Normal cerebral function
Stable cardiac rhythm
Perfusion good enough to maintain organ function
Good quality of life
Most common cause of death for patients surviving to ITU
Brain injury
Targeted temperature management
32-36 degrees for 24 hours
Followed by rewarming at a rate of 0.25 degrees per hour
Complications of hypothermia
Arrhythmias - bradycardia
Increased infection rates
Hyperglycaemia
Diuresis and electrolyte abnormalities - hypophosphataemia, hypokalaemia, hypomagnesaemia, hypocalcaemia
Impaired coagulation
Increased amylase concentration
Reduced clearance of sedative drugs
When should neurological outcome be assessed
At least 72 hours after ROSC using a structured multimodal approach
