What is postpartum depression
Sx start during pregnancy or w/in 4 weeks of delivery
5+ mood and cognitive Sx for 2 consecutive weeks (1 must be depressed mood or loss of interest in pleasure)
What are postpartum blues
Sx start 2-3 days after delivery and RESOLVE w/in 2 weeks of onset
mild, self limited Sx
Don’t need a minimum # of Sx to diagnose!
RF for postpartum depression are
Hx of depression increased post-natal stress Hx of PPD <25 y/o unintended or unwanted pregnancy poor self body image FHx of depression
Cause of postpartum depression is
UNKNOWN!
may be w/w genetics or hormonal changes
How do you screen for postpartum depression
Edinburgh postnatal depression scale
-this is to be done in all postpartum women regardless of Sx, 4-8 weeks after delivery
What is the Edinburgh postnatal depression scale
10 questions
add numbers associated with each question
Not used to diagnose, just to screen
<10= Low risk
Symptoms of depression include
- Depressed mood most of day, nearly every day
- Loss of interest or pleasure in most activities
- Insomnia or Hypersomnia
- Significant weight loss or gain (5% in 1 mo)/ decreased appetite
- Psychomotor retardation or agitation observable by others
- Fatigue, low energy
- Decreased ability to concentrate
- Thoughts of worthlessness or guilt
- Recurrent thoughts of death or SI
