Q

Why wouldn’t you prescribe Pethidine?

A

Pethidine use is strongly discouraged because of its short duration of action, accumulation of the toxic metabolite norpethidine which may result in seizures, and its high propensity for inducing drug seeking behaviour.

Q

Why wouldn’t you use metoclopramide or droperidol to treat a patient with parkinson’s disease?

A

Because they both block dopamine receptors. Reduced dopamine levels is involved in the pathogenesis of Parkinson’s disease.

Q

In which cohort of patients is codeine ineffective?

A

Codeine is converted to morphine by CYP2D6 and has little or no analgesic activity on it’s own, so it will be ineffective in CYP2D6 ‘poor metaboliser’ patients.
