Late diagnosis of HIV
CD4 count <350 cells/mm3
Offer HIV testing if prevalence is
> 2 per 1000
Example of indication for HIV testing
Chronic unexplained diarrhoea
PCP
Pneumocystis (jiroveci) pneumonia
Test for PCP
Broncho-alveolar lavage (BAL)
CD4 count for which HIV infected people are vulnerable to PCP
<200 cells/mm3
Prevent PCP in HIV +ve patients
PCP prophylaxis:
1st line - low dose co-trimoxazole 960mg OD
2nd line - Dapsone 50mg OD
3rd line - Atovaqone 750mg BD
If HIV +ve, who else to test
Sexual contacts
Children of a woman +ve for HIV
Baby HIV testing
Birth, 6 and 12 weeks
- HIV DNA PCR
18 months
- HIV antibody
Initiating HAART can result in
Immune reconstitution inflammatory syndrome (IRIS)
so if serious opportunistic infections, postpone HAART as IRIS can worsen outcomes
