Definition
Positioning therapy can help match ventilation & perfusion through the redistribution of oxygen & blood flow in the lungs, which improves gas exchange
> the least damaged portions of lungs receive preferential blood flow, resulting in less V/Q mismatch
Prone Positioning
- Turning the pt completely over onto stomach, face down
- Used to improve oxygenation in pts w/ ARDS
- Can be used to facilitate the mobilization of secretions & provide pressure relief
Why Prone for ARDS
Dependent lung areas are more heavily damaged than nondependent lung areas. Turning the pt to prone improves perfusion to less damaged areas of lungs, improves V/Q match, & dcrs intrapulmonary shunting
Prone Position - Contradications
- Pts w/ incrd intracranial pressure
- Hemodynamic instability
- Spinal cord injuries
- Abdominal surgery
Rotation Therapy
Improve oxygenation through better matching of ventilation to perfusion & to prevent pulmonary complications associated w/ bed rest & mechanical ventilation
Continuous Lateral Rotation Therapy (CLRT)

Pt is continuously turned from side to side w/ a rotation of 40 degrees or greater/less than
> Kinetic Bed: the entire platform of the bed rotates
> Oscillation Bed: the mattress inflates & deflates to provide rotation
