A

Positioning therapy can help match ventilation & perfusion through the redistribution of oxygen & blood flow in the lungs, which improves gas exchange
> the least damaged portions of lungs receive preferential blood flow, resulting in less V/Q mismatch

Prone Positioning

A
  • Turning the pt completely over onto stomach, face down
  • Used to improve oxygenation in pts w/ ARDS
  • Can be used to facilitate the mobilization of secretions & provide pressure relief

Why Prone for ARDS

A

Dependent lung areas are more heavily damaged than nondependent lung areas. Turning the pt to prone improves perfusion to less damaged areas of lungs, improves V/Q match, & dcrs intrapulmonary shunting

A
  • Pts w/ incrd intracranial pressure
  • Hemodynamic instability
  • Spinal cord injuries
  • Abdominal surgery

A

Improve oxygenation through better matching of ventilation to perfusion & to prevent pulmonary complications associated w/ bed rest & mechanical ventilation

Continuous Lateral Rotation Therapy (CLRT)

A
  • Pt is continuously turned from side to side w/ a rotation of 40 degrees or greater/less than
    > Kinetic Bed: the entire platform of the bed rotates
    > Oscillation Bed: the mattress inflates & deflates to provide rotation
